Job Description
About the Role
Provide direct patient care, assess client needs, and develop and implement treatment plans. Collaborate with interdisciplinary teams to ensure clients receive appropriate services.
Responsibilities
- Conduct intake assessments and diagnose clients’ mental health needs
- Develop and implement treatment plans, Goals and objectives, and discharge plans
- Provide therapeutic interventions and group therapies
- Document client progress and maintain accurate records
- Collaborate with interdisciplinary teams to coordinate care
Requirements
- Master’s degree in Social Work or a related field
- Clinical licensure as a Social Worker in NY
- Previous clinical experience preferred
- Excellent listening, communication, and interpersonal skills
- Strong diagnostic and evidence-based intervention skills
