About the Role

Responsibilities
- Assess clients’ needs and develop individualized care plans
- Provide counseling and support to clients and their families
- Connect clients with community resources and services
- Advocate for clients’ rights and access to services
- Maintain accurate and confidential client records
- Coordinate with other healthcare and social service professionals
- Conduct home visits and assessments as needed
- Participate in professional development and continuing education
Requirements
- Bachelor’s degree in Social Work or related field
- State licensure as a Social Worker (LCSW or equivalent)
- Previous experience in social work or a related field preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassion and empathy for clients and their families
- Knowledge of community resources and social service programs
- Commitment to ethical practice and client confidentiality
