About the Role
Provide crisis intervention and resource coordination for at-risk populations through Philadelphia Social Services Network. Manage caseload of 50+ clients with complex needs.
Responsibilities
- Conduct needs assessments
- Develop service plans
- Coordinate with agencies
- Maintain case documentation
- Advocate in court hearings
- Provide trauma-informed care
- Lead community workshops
Requirements
- BSW or related degree
- 2+ years case management
- Knowledge of Medicaid/SNAP
- Valid PA driver’s license
- CPR/First Aid certified
- Evening availability
- Clear child abuse history
