About the Role
Compassionate Hearts Hospice seeks a licensed Social Worker to provide emotional support and resources to patients and families during end-of-life care. This meaningful role makes a difference in people’s most vulnerable moments.
Responsibilities
- Conduct psychosocial assessments
- Provide counseling and crisis intervention
- Coordinate community resources
- Advocate for patient needs
- Document care plans and interventions
- Participate in interdisciplinary team meetings
Requirements
- Master’s in Social Work (MSW)
- Current state LMSW or LCSW license
- 2+ years experience in healthcare
- Knowledge of Medicare/Medicaid guidelines
- Excellent communication skills
- Valid driver’s license and reliable transportation
