Provide critical support to individuals and families in need through resource coordination, crisis intervention, and advocacy.
Responsibilities
- Conduct client intake assessments
- Develop service plans
- Connect clients with housing/food resources
- Monitor progress via home visits
- Document case files
- Collaborate with community partners
Requirements
- Bachelor’s in Social Work
- 1+ years case management
- Knowledge of local welfare programs
- Valid driver’s license
- Empathy and cultural sensitivity
- Bilingual (Spanish) preferred
