About the Role
Provide case management support for vulnerable families at Metropolitan Family Services. Conduct assessments, connect clients with resources, and advocate for community needs. Serve diverse populations in Philadelphia neighborhoods.
Responsibilities
- Maintain caseload of 20+ families
- Develop individual service plans
- Connect clients to housing and employment resources
- Coordinate with schools and community organizations
- Document all client interactions thoroughly
- Participate in multidisciplinary team meetings
- Conduct home visits and follow-up sessions
Requirements
- MSW or BSW from accredited program
- State social work licensure (LBSW or equivalent)
- 2+ years experience in human services
- Knowledge of community resources in Philadelphia
- Cultural competence with diverse populations
- Valid driver’s license and reliable transportation
- Computer literacy in case management systems
