About the Role
We’re recruiting for experienced LPNs to provide direct care within our skilled nursing communities. This mid-level role requires strong patient assessment skills, medication administration oversight, and collaborative communication with Registered Nurses. Must be adaptable to senior care environments.
Responsibilities
- Perform patient assessments and documentation
- Administer medications and monitor patient reactions
- Coordinate care plans with RNs and therapists
- Assist with wound care, dressing changes, and injections
- Participate in patient and family education for discharge planning
- Maintain HIPAA compliance through strict records management
- Ensure infection control standards during procedures
- Participate in weekly staffing reviews for care coordination
Requirements
- Current LPN license in state of practice
- Minimum of 2 years’ experience in long-term care settings
- BLS and ACLS certification active or within 90 days
- Strong communication skills for family conferences
- Knowledge of geriatric care principles
- Ability to lift patients when necessary and perform repositioning
- Adaptability to flexible rotating shifts
- Proficient with EMR/EHR software systems
