Job Description
About the Role
Provide skilled nursing care for residents in our rehabilitation center. Collaborate with RNs, CNAs, and therapists to develop and implement individualized care plans that restore function and improve quality of life.
Responsibilities
- Perform initial assessments and regular re-evaluations of resident health status
- Administer medications and document treatment protocols
- Monitor vital signs and patient progress during physical therapy and rehabilitation
- Communicate patient care changes or concerns to the care team
- Complete detailed documentation of patient conditions and interventions
- Educate patients/families on injury prevention, health maintenance, and treatment plans
- Maintain equipment, supplies, and patient records according to facility policies
Requirements
- Active LPN license in good standing
- Graduate of an accredited nursing program
- 2+ years experience in long-term care/rehabilitation facility preferred
- Current CPR, BLS (Basic Life Support) certification
- Strong communication skills with patients, families, and care teams
- Ability to pass federal and state background checks
- Flexibility to work varying shifts including weekends and holidays
