About the Role
Community Care Services is seeking a dedicated Social Worker to support individuals and families in need. You will assess client needs, develop care plans, and connect them with community resources to improve their well-being.
Responsibilities
- Conduct assessments to evaluate client needs
- Develop and implement individualized care plans
- Provide counseling and crisis intervention
- Connect clients with community resources and services
- Advocate for clients’ rights and needs
- Maintain accurate and confidential client records
Requirements
- Bachelor’s degree in Social Work or related field
- State licensure as a Social Worker (LSW or LCSW)
- Experience working with diverse populations
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of community resources and social services
