About the Role
As a Social Worker at Community Care Services, you will provide support and assistance to individuals and families in need. You will assess clients’ needs, develop care plans, and connect them with resources to improve their quality of life.
Responsibilities
- Assess clients’ needs and situations
- Develop and implement care plans
- Provide counseling and support to clients
- Connect clients with community resources
- Maintain accurate case records
- Collaborate with other social service agencies
- Advocate for clients’ rights and needs
Requirements
- Bachelor’s degree in Social Work or related field
- Licensed Social Worker (LSW) in the state of California
- 1+ years of experience in social work
- Strong interpersonal and communication skills
- Ability to work independently and in a team
- Empathy and understanding of diverse populations
- Valid driver’s license and reliable transportation
