About the Role
Beacon Public Services needs a Case Manager to support at-risk youth. Connect clients with housing, employment, and mental health resources.
Responsibilities
- Maintain caseload of 25-30 clients
- Conduct needs assessments
- Develop service plans
- Coordinate with community partners
- Document client progress
- Crisis intervention
Requirements
- Bachelor’s in Social Work
- 2+ years case management
- Valid driver’s license
- Trauma-informed care training
- Bilingual (English/Spanish)
- LCSW eligible
