About the Role
Community First Services seeks a licensed social worker to provide case management for at-risk populations. Conduct assessments and connect clients with resources.
Responsibilities
- Maintain caseload of 30+ clients
- Develop care plans
- Coordinate with agencies
- Document interactions
- Lead support groups
- Advocate for client needs
Requirements
- LCSW certification
- 2+ years field experience
- Knowledge of CO Medicaid systems
- Valid driver’s license
- Bilingual (English/Spanish) preferred
- Trauma-informed care training
