About the Role
Provide compassionate care to patients in a long-term care facility. Assist with daily living activities, administer medications, and collaborate with healthcare professionals.
Responsibilities
- Monitor vital signs and record patient health data
- Assist with personal hygiene needs, dressing, and bathing
- Administer prescribed medications and treatments
- Collaborate with RNs, doctors, and PT for patient care planning
- Educate patients and family members on care and wellness
- Maintain accurate medical records and documentation
Requirements
- Current LPN licensure in the state of Ohio
- Graduate from an accredited Practical Nursing program
- 1-3 years nursing experience in long-term care preferred
- Effective communication and time-management skills
- Ability to handle physically demanding tasks
- Strong ethical standards and patient confidentiality compliance
