About the Role
Provide critical support to families in need through resource coordination and advocacy. Manage caseload of 25+ clients, connecting them with housing, employment, and healthcare services.
Responsibilities
- Conduct client assessments
- Develop service plans
- Maintain case files
- Coordinate with agencies
- Crisis intervention
- Advocate in court
- Track program outcomes
- Attend community meetings
Requirements
- Bachelor’s in Social Work
- 2+ years case management
- Valid state license
- Cultural competency
- Crisis management skills
- Bilingual (Spanish)
- Valid driver’s license
- Clear background check
