About the Role
Coordinate wraparound services for unhoused individuals through street outreach and shelter partnerships. Connect clients with housing resources, mental health services, and employment programs.
Responsibilities
- Conduct needs assessments
- Develop service plans
- Maintain case documentation
- Collaborate with NGOs
- Facilitate group workshops
- Track program outcomes
Requirements
- BSSW or related bachelor’s degree
- 2+ years case management
- HUD-VASH experience
- Crisis intervention training
- Valid driver’s license
- Bilingual (Spanish)
