About the Role
Join our skilled nursing team! We have an opening for an LPN who thrives in a patient-focused environment. This role focuses on providing direct care to residents while working collaboratively with RNs and MDs.
Responsibilities
- Perform routine clinical tasks and vital sign monitoring
- Assist with resident admissions, discharges, and transfers
- Administer prescribed medications per protocols
- Document patient care services provided
- Communicate health changes to MDs
- Maintain a clean, safe environment for residents
Requirements
- Licensed Practical Nurse (LPN) degree
- Active LPN license
- 3+ years SNF experience preferred
- Strong clinical assessment skills
- Excellent communication abilities
- Ability to work independently or as part of a team
