About the Role
We are seeking a compassionate and organized Case Manager to provide supportive services to clients facing challenging life circumstances. You will assess clients’ needs, develop service plans, and link them with appropriate community resources.
Responsibilities
- Conduct comprehensive assessments of clients’ needs
- Develop and implement individualized service plans
- Coordinate with community agencies to provide comprehensive support
- Maintain accurate records and case notes
- Provide crisis intervention and emotional support to clients
- Advocate for clients and assist them in navigating system barriers
Requirements
- Bachelor’s degree in Social Work, Human Services, or a related field
- Proven experience in case management or a related field
- Strong knowledge of community resources and social services
- Excellent assessment, planning, and organizational skills
- Strong communication and interpersonal skills
