About the Role
City Health Clinic seeks an LPN to provide direct patient care under RN supervision. Conduct assessments, administer medications, and maintain EHR records in our fast-paced community health setting.
Responsibilities
- Perform vital sign monitoring
- Administer immunizations and medications
- Assist with wound care and specimen collection
- Document care in Epic EHR system
- Educate patients on chronic disease management
- Coordinate referrals to specialists
- Maintain infection control protocols
Requirements
- Active PA LPN license
- 2+ years clinical experience
- BLS certification
- Proficiency with EHR systems
- Spanish language proficiency
- Experience with pediatric populations
- Flexibility for evening shifts
