Job Description
About the Role
Provide direct patient care under supervision of RNs and MDs in our 20-bed skilled nursing facility. Collaborate with nursing staff to deliver compassionate care to short-term rehabilitation and long-term residents.
Responsibilities
- Monitor and record patient vital signs and medical information
- Administer injections, medications and treatments per nursing orders
- Assist with turning, transferring and ambulating patients
- Document patient care records including intake/output and conditions
- Collaborate with social work, PT and OT staff to support patients
- Attend daily multidisciplinary team meetings and case conferences
- Order and stock nursing supplies and equipment
- Maintain infection control and sanitation procedures
Requirements
- Current LPN license in good standing with the state board of nursing
- Graduate of accredited Practical Nurse program
- Basic Life Support (BLS) certification through AHA
- 1+ year of long-term care or acute care nursing experience
- Strong organizational, verbal and written skills
- Empathetic, reliable personality with strong attention to detail
- Ability to perform CPR and physical interventions if needed
- Comfortability with EMR documentation in Cerner/Epic
