About the Role
Boston Community Health Center seeks a Licensed Practical Nurse to provide compassionate patient care in diverse community settings. You’ll work with medical teams to deliver essential nursing services to underserved populations. This role directly impacts patient outcomes through daily care coordination and health education.
Responsibilities
- Perform patient assessments and report changes to RNs or physicians
- Administer medications and treatments per established protocols
- Document patient care activities in EHR systems accurately
- Assist with routine examinations and therapeutic procedures
- Educate patients on health maintenance and disease prevention
- Monitor vital signs and recognize emergent conditions
- Coordinate patient admission and discharge processes
Requirements
- Valid LPN license in Massachusetts
- Graduate of accredited practical nursing program
- Minimum 1-year clinical experience
- CPR/BLS certification
- EHR system proficiency (Epic preferred)
- Ability to navigate fast-paced environments
- Bilingual in Spanish or Haitian Creole preferred
