About the Role
Support our public service agency as a Case Manager to assess client needs, develop service plans, and coordinate resources to meet those needs.
Responsibilities
- Conducts comprehensive assessments of clients’ needs and circumstances
- Develops and implements individualized service plans to meet client needs
- Coordinates with internal and external agencies to access resources and services
- Maintains accurate and up-to-date client records and progress notes
- Monitors client progress and evaluates the effectiveness of service plans
Requirements
- Bachelor’s degree in Social Work, Human Services, or a related field
- Previous experience in case management or human services
- Strong communication, active listening, and problem-solving skills
- Ability to work effectively with diverse populations and cultures
- Competency in using case management software and databases
