About the Role
Provide direct patient care at Cedar Ridge Rehabilitation Center under RN supervision. Monitor vital signs, administer medications, and document patient progress.
Responsibilities
- Perform wound care and dressing changes
- Assist with mobility and ADLs
- Monitor and report changes in condition
- Maintain medical records
- Administer prescribed medications
- Collaborate with interdisciplinary team
Requirements
- Active Illinois LPN license
- CPR/BLS certification
- 1+ years clinical experience
- Knowledge of EHR systems
- Physical stamina for 12-hour shifts
- TB test clearance
