Community Care Network seeks a coordinator to connect low-income families with housing, healthcare, and employment resources. Bilingual English/Spanish required. Field work involved.
Responsibilities
- Assess client needs
- Maintain case files
- Organize community workshops
- Collaborate with agencies
- Track program outcomes
- Attend neighborhood meetings
Requirements
- BSW or related degree
- 2+ years case management
- Bilingual English/Spanish
- Valid driver’s license
- Knowledge of benefit programs
- Crisis intervention training
