About the Role
Provide compassionate, high-quality nursing care to residents in an assisted living and skilled nursing facility. Monitor patient health, administer medications, and collaborate with interdisciplinary teams to optimize resident wellbeing.
Responsibilities
- Conduct comprehensive assessments of residents’ health conditions and create individualized care plans
- Administer medications, treatments, and basic wound care as ordered by the physician
- Monitor vital signs, track changes in medical status, and report abnormalities to the nursing supervisor
- Collaborate with doctors, nurses, and therapists to evaluate and modify patient care plans
- Educate families on resident care needs and discharge planning
- Maintain accurate and complete medical records, documenting all care provided
Requirements
- Current and active LPN license in the state of Pennsylvania
- Graduate of an accredited LPN program with minimum of 1-year experience in long-term care
- Certification in Basic Life Support (BLS) required
- Demonstrated ability to communicate effectively with patients, families, and staff
- Strong critical thinking and problem-solving skills
- Experience with electronic health record (EHR) systems preferred
