About the Role
We are seeking a dedicated Community Health Worker to promote health and wellness within our community. This role involves educating individuals and groups about health issues, providing resources, and linking them to appropriate healthcare services.
Responsibilities
- Educate individuals and groups about health issues, including preventive care and wellness
- Provide culturally sensitive and appropriate health information and resources
- Assist individuals in navigating the healthcare system and accessing needed services
- Conduct outreach activities and health screenings in community settings
- Collaborate with healthcare providers and community organizations to improve health outcomes
- Document and report on client interactions and outcomes
- Maintain a safe and supportive environment for clients
- Promote community health initiatives and programs
Requirements
- High school diploma or equivalent
- Certification as a Community Health Worker (CHW) or equivalent (preferred but not required)
- Previous experience in health education or community outreach
- Excellent communication and interpersonal skills
- Strong organizational and time management skills
- Knowledge of community resources and healthcare systems
- Ability to work independently and in a team setting
- Fluent in one or more languages spoken in the community (preferred)
